First visit?
Please bring a list of all medications you are taking, and how you are taking them. If possible, please bring pertinent medical records, or have them sent to us from yourprevious physician.
Insurance Questions?
LMA participates in most area health plans, but check to be sure your insurance plan is on our list of participating plans. LMA requests payment for all co-pays at the time of your visit.
